Making a Measurable Difference in the lives of our clients each day

Do you have what it takes to be a Communities of Care nurse?

We serve medically fragile children, teens, and adults who live with their families. Our nurses provide extended-hours skilled care in clients’ private homes in and around the 7-county Twin Cities metro area.

With our unique staffing and scheduling method, each nurse usually works with just one case. This allows nurse and client to establish a high level of trust–and that leads to better care. Every nurse has the opportunity to meet the client as part of the placement process. When both client and nurse agree to the match, everyone feels more comfortable.  (read more here )

Successful home care nurses work confidently and autonomously to assess their clients’ medical needs and respond appropriately within the established plan of care. Nurses must work as both compassionate caregivers and skilled professionals.

We are looking for LPNs and RNs who share the core Communities of Care values:

Clinical Excellence:  Our nurses provide medical attention in line with best practices and their clients’ plan of care.
Caring Professionals:  Our nurses understand how to care for the whole client in a home setting while maintaining appropriate professional boundaries.
Customized Care:  Our nurses understand the value of honoring client preferences regarding care, staffing, and scheduling. Our nurses work to ensure that our clients feel comfortable in their own homes.
High Ethical Standards:  Our nurses are trained on documentation standards which ensure a comprehensive, accurate medical record for the client and adherence to the highest standards for ethical business practices.
