At Communities of Care, we take pride in our inclusive approach to home care. While the home care dynamic between nurse and client is primary, we want to involve the family and physicians in all aspects of the care plan.

We build our block schedules to guide routines clearly for both nurses and families. Nurses know precisely when they work, and families can rely on all team members to work together. We encourage parents and other caregivers to play an active role in creating an excellent environment for their loved ones.

We also work closely with our clients’ physicians and other care providers to ensure the best treatment plans. Our records are always up to date and reflect changes in medication and cares as soon as possible to help everyone stay on the same page.
