Communities of Care has one team of easily accessible office staff, and we make it a point to promptly respond to every nurse and client inquiry with a professional and caring tone.
When I started this business in 1999, I noticed a need for consistency and personalized treatment in the Minnesota home care space. We have always been committed to a responsive, collaborative approach to home care, and we are proud to set an industry standard.
Our staffing director and Director of Nursing are both hands-on throughout the interview and orientation process. We want to make sure our nurses are informed and feel thoroughly prepared. To help this process, our comprehensive training documents are written with reader ease in mind, not legalese.
Once the client and family are settled into a routine, our office staff are available to work through any issues that might come up. We note every issue or incident and document it thoroughly, including follow-up conversations and remedial actions. We work with the client, the nurse, the family, and other members of the care team to ensure a smooth workflow and a collaborative team!
